‘Transition’ is the term that is used when people are preparing to move from children services to adult services. This can be an anxious time for the individual and also family members.
Ideally transition planning would begin after a child turns 14 and a transition plan would be created, with input from the child and relevant family members.
Workshops to help with transition
Transition plans for children with autism will usually cover issues such as:
* education and training * health * housing * transport * work * relationships * hobbies, sport and leisure activities
To compliment the transition survey Healthwatch are hosting two workshops for anyone that has been through or is currently going through the transition period, either as a family member or an individual with Autism.
Where and when
The workshops are taking place at Winchester House, Shanklin on 18 March 2015 from 6pm – 8pm or at Downside Community Learning Centre, Newport on 25 March 2015 from 10am – 12pm. There is no need to book, simply turn up.
Your feedback can help influence and improve services on the Island. The survey can be accessed by visiting our website or calling 01983 608608. Hard copies of the survey are available upon request. The survey now closes on 31 March 2015.
