Report on death of two day old baby published

A report has been published today (Wednesday 15 October) by the Isle of Wight Safeguarding Children Board following a serious case review relating to the death of a two day old baby.

The independent review highlights areas of practice by various agencies that could be improved however the report author stresses that in this case it cannot be said with any degree of certainty that changes in practice would have resulted in a different outcome.

Twelve recommendations
Recommendations for improvement are detailed in the published report.

Chair of the Isle of Wight Safeguarding Children Board, Maggie Blyth, commented:

“On behalf of all of the agencies involved in this review, I would like to express our sadness at the tragic death of this baby and offer our condolences to the family.

“All agencies have a responsibility to protect vulnerable children and their involvement in this case has been reviewed by an independent panel to examine their response, how they have worked together and whether more could have been done to protect the child involved.

“Although we can’t be certain that changes in practice would have prevented this baby’s death, the report does focus on a number of areas of learning for the agencies involved, particularly in relation to the way that information is passed between professionals and different agencies. The report also highlights the need to review how agencies work together in the ‘rapid response’ provided following a baby’s death.

“The Isle of Wight Safeguarding Children Board will be working closely with all of the agencies involved to ensure the recommendations from this serious case review are taken forward without delay.”
